Job Context:
- SAJDA Foundation is seeking an Voucher Scheme Field Worker (VSFW) for its PROSHOMON Project based in Dhaka (with frequent travel to Project sites), Bangladesh. The project will focus to improve the health and nutrition status of the extreme poor by increasing accessibility, coverage, comprehensiveness and sustainability of existing primary health and nutrition services. The project will contribute in the government commitments to reach SDGs target by increasing the coverage of Universal Health Coverage (UHC) in the urban area of Bangladesh. Ensure better accessibility of the urban extreme poor segment by using technology and innovation in health financing scheme in the focus of this project.
- The Project will work with 12,000 extreme poor households in order to improve their health and nutrition status by increasing accessibility, and utilize free Essential Service Packages (ESP of quality primary health and nutrition care. The VSFW are the most frontline staff engaging with the beneficiaries on the use of voucher scheme. Each VSFW will be responsible for 1000 households.
- Under the direct supervision of the Project Head, the Voucher Scheme Field Worker (VSFW) will undertake the following responsibilities and tasks as necessary related to the project.
Job Description / Responsibility:
- Identification of extreme poor households and selection of beneficiaries based on the given criteria
- Collection of household data with detailed information of all the members of the households
- Along with supervisors (Field Coordinator of SF, Concern Coordinator) a final list of enlisted households
- Visit all the selected targeted households at regular intervals to orient households on voucher scheme benefits available to their use of SMART Card of the facility and the packages of services
- During household visits VSFW will identify malnourished children, eye problems and non-communicable diseases and refer them to health facilities and follow-up. Maintain a record of services used
- With support from ICT unit VSFW will take photographs and finger prints of the family members for the software
- Make a list of health facilities available(Government facilities, NGO/Private facilities) according to Project Service Package using Project Criteria
- Distribution of SMART Card on preannounced and pre-scheduled date along with senior staff
- Orient all the households on voucher scheme benefits available to their how to use the SMART Card at the facility and explain processes
- Maintain a register of the services used by beneficiaries and claims submitted by the facilities
- Random visit to selected facilities to verify if the beneficiaries received services according to their need
- Conduct a survey to identify a preliminary list of eligible health facilities for empanelment and to sign contract to work collaboratively outlining the clear roles and responsibilities of both parties
- Map services provided by eligible health facilities making an assessment of them using the project criteria
- Cooperation and collaboration with Upazilla Health Complexes and municipalities using a structured form
- Provide information to all SMART Card holders on the location, opening hours and availability of services of the empanelled health facilities
- Monitor and verify voucher utilization and claim submission for reimbursement by the empanelled facilities
Job Nature: Full-time.
Educational Requirements: Nursing Diploma from Bangladesh Nursing Council/ Paramedic Course from a recognized institution.

- Age 18 to 39 year(s)
- Minimum 3 years working experience in health programs especially at the community level
- Self motivated, able to work independently and as part of a multidisciplinary team
- Experience in public health, expertise in working with data for reporting will be advantageous
