• The International Rescue Committee (IRC) responds to the world`s worst humanitarian crises and helps people to survive and rebuild their lives. Founded in 1933, the IRC offers lifesaving care and life-changing assistance to refugees forced to flee from war or disaster.
• Background of Country and Emergency Response:
• Since August 25, 2017 more than 671,000 people have escaped from Myanmar into Cox Bazar, Bangladesh. As Monsoon rains being in March, the population in the refugee camps are at a high risk of floods and landslides. More than 102,00 refugees are at risk of being directly impacted by these natural disasters. Along with monsoon rains, tropical cyclones are prone to this area from March to October, which could bring catastrophic damage and loss of life to both host and refugee populations due to lack of properly constructed shelters.
• Along with the risk to the refugee and host population, there is risk that health facilities will be inaccessible or damaged by the weather events. In response to this threat, IRC is creating Mobile Medical Teams which will be dedicated to responding to affected areas by providing lifesaving emergency and primary health care to the population.

Job Responsibilities:

• JOB OVERVIEW/SUMMARY:
• The Nurse/Paramedic is a member of the IRC health team and will be responsible for supporting the implementation of the IRCs Primary healthcare activities in the Rohingya Refugee Camp, Cox Bazar after natural disaster events such as floods, landslides and cyclones. (S)He will ensure the delivery of high quality emergency and primary healthcare activities to the local catchment population including child health and sexual and reproductive healthcare services. Whilst the MMT is not deployed, the team will contribute to improving the IRC's existing health programs in the camps.
• Under the supervision of the Medical Doctor and in-conjunction with other healthcare staff, the Nurse/Paramedic will be responsible for the delivery of basic nursing care, and in particular:
• Apply nursing knowledge and skills to the prevention, diagnosis, and treatment of emergency and primary healthcare problems, ensuring appropriate patient triage and referral as needed.
• Provide health promotion and counselling to patients and caregivers.
• Ensure emergency and primary healthcare services are implemented in conformity with relevant Ministry of health or international guidelines.
• Ensure timely and accurate recording of patient information and consultations in relevant patient registers and facilitate the timely submission of require health reports.
• MAJOR RESPONSIBILITIES:
• CLINICAL CARE
• Provide direct clinical care, working closely with the other mobile team clinical staff to ensure the delivery of high quality emergency and primary healthcare services.
• Triage, screening and assessment of patients.
• Provision of inpatient and outpatient treatment, ensuring medications are given to patients as prescribed by the Medical Doctor.
• Delivery of hygiene promotion, health education and patient/care giver counselling.
• Ensure adherence to infection prevention and control measures.
• Ensure accurate recording of information in the clinic registers, and timely completion of summary reports.
• Ensure clinic areas are appropriately stocked with essential equipment and supplies.
• Ensure patient confidentiality is respected, and that all consultations are conducted in a dignified and respectful manner.
• STAFF SUPERVISION & DEVELOPMENT
• Maintain open and professional relations with team members, promoting a strong team spirit and provide guidance to enable staff to successfully perform in their positions.
• Provide nursing/medical support and mentorship to other mobile medical team staff, providing on-the-job training where needed.
• PROJECT MANAGEMENT
• Implement outreach activities with the rest of the team to access "hard to reach? and vulnerable beneficiaries to increase service utilization.
• When available work with and support community volunteers to provide health promotion, community mobilization and awareness raising.
• Ensure regular inventory and consumption tracking of medical drugs and supplies.
• Work with relevant staff to ensure prompt requesting of medical drugs and supplies.
• COORDINATION & REPRESENTATION
• Develop and maintain effective working relationships with local communities, local government agencies and other implementing partners.
• MONITORING & REPORTING
• Ensure all patient consultations are documented and patient records completed.
• Submit daily reports in a timely manner.
• OTHER DUTIES
• Consistently and proactively monitor/assess the safety and security of the team; promptly reporting concerns or incidents to IRC management and liaising with community leaders and other external parties as required to maintain/enhance the security environment for IRC programs.
• PROFESSIONAL STANDARDS:
• All IRC staff are required to adhere to THE IRC Way Standards for Professional Conduct and the IRC country employment policies.
• WORK ENVIRONMENT:
• The Mobile Medical Team will be working in various locations within the Rohingya Refugee Camp in the Ukhia district and will be working in open environments and required to set up a mobile work station.
• Mobile Medical Teams will deploy to affected areas after natural disasters such as floods, landslides and cyclones; therefore work environments will change frequently.
• Mobile Medical Teams may be required to carry supplies in backpacks, walk to affected areas and set up work stations.

Employment Status: Full-time.

Educational Requirements:

• Qualified Nurse with a Nursing diploma/certificate or Qualified Paramedic with Paramedic diploma/certificate.
• Registered with local Nursing/Paramedic Council.

Experience Requirements: At least 2 year(s).

Additional Requirements:

• At least 2 years clinical Nursing/Paramedic experience in emergency and primary healthcare.
• Previous experience and training in sexual gender-based violence and clinical care for sexual assault survivors (desirable)
• Ability to lead, train, supervise and facilitate other medical staff and community health workers in their respective tasks.

• Skills to provide sensitive and appropriate medical counselling.

• Provide quality STI and GH services to the MSM and hijra at service centers, which include DICs, Sub-DICs, Outlets and Satellites.
• Provide HTS services to the MSM and hijra at service centers.
• Conduct physical examinations of the beneficiaries maintaining proper infection prevention procedure and ensure privacy during physical examination.
• Record present and past medical complaints of beneficiaries properly in the health card and prescribe medicines as per the treatment protocol; ensure proper dispensing of drugs including DOTs to beneficiaries of the service centers and maintain records accordingly.
• Provide prescriptions for STIs for the female partners of MSM
• Ensure availability of proper clinical set up (e.g. patient bed, spot light, patient screen, adequate lighting facilities, running water supply etc.), infection prevention equipment (autoclave, incinerator, etc.) and clinical equipment (e.g. BP machine, thermometer, weight machine etc.). Ensure that they are in operation and properly used.
• Ensure that adequate medicines are kept in the store in proper environment as per the standard practice; update medicine stock register regularly, review medicine stock, notify regarding any stock-out/short expiry situation to concerned authority at least four (4) months back and prepare medicine requisition when required.
• Ensure ethical issues like confidentiality, non-discriminatory attitude towards MSM and hijra populations are maintained; and treat beneficiaries with respect and dignity
• Store the blisters of DOTs medicine (i.e., Cefixime and Azythromycine) at service centers until the verification by PR or SR representatives. Keep records of incineration of the blisters after the verification by the PR or SR.
• Ensure proper application of all monitoring tools for monitoring clinical activities (STI, GH, TB) and HTS services; Review overall clinic performance regularly and guide the staff and report to the concern Programme specialist/ Programme coordinator.
• Provide counseling assistance in regards to personal risk assessments, safer sex behaviors and partner notification; perform 4Cs (i.e. compliance with treatment, counseling for prevention, condom demonstration, contact tracing) according to National as well as project guideline during counseling; demonstrate proper use of condoms;
• Perform TB screening of all patients attending the clinic; ensure accompanied referral of suspected TB cases to DOTS center for diagnosis and/or treatment and follow-up of TB positive cases to ensure treatment adherence.
• Perform HTS activities as per schedule; perform pre and post-test counseling, collect sample from HTS clients, perform rapid HIV testing according to algorithm, deliver report to the clients, keep record maintaining confidentiality; maintain appropriate bio-safety measures during testing in order to ensure quality of test, accompanied referral of HIV positive clients to the care, support and treatment center (CSTC).
• Prepare list of requisition for HTS reagent and consumables; maintain stock register of HTS reagent and consumable regularly.
• Store and document QC samples to be send to the icddr,b laboratory; store reagents and samples maintaining cold chain in order to organize and preserve samples properly.
• Ensure proper registration of clinic patients on each clinic day and maintain all relevant records and files; submit monthly report to the DIC Manger. Prepare monthly HTS report and submit to SR focal person timely.
